NCQA
The National Committee on Quality Assurance (NCQA) evaluates and awards Medical Home Recognition to practices when they meet its stringent guidelines. We are proud to have achieved NCQA's highest level of accreditation, Level III.
"Patient Centered Medical Home facilitates partnerships between individual patients, and their provider, and when appropriate, the patient's family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner."

Diabetes Recognition Program Award
This achievement recognizes us for providing the highest-level of diabetes care to our patients. NCQA, in partnership with the American Diabetes Association (ADA), has developed this voluntary program to reward those providers who use evidence-based measures and provide excellent care to their patients with diabetes.
New York State Health Foundation Grant
We received a significant award to improve the care of elderly diabetics by spreading the team approach to patient care across the entire Health Center.
United Hospital Fund
This grant was awarded to design and implement a comprehensive education curriculum for our medical assistants.
September 11th Fund
Awarded grant to create a Disaster Benefits Unit to work intensively with displaced UNITE union members and their families to enroll them in state funded health programs.
NYS Attorney General's Office
We were awarded a grant to improve nutrition education for low wage immigrant workers.
